Working Musician Health Insurance: The 2026 Liner Notes

May 14, 20269 min readBy Stage & Sound Health

Health insurance for working musicians is its own niche, and the standard advice — 'just go on healthcare.gov and pick a plan' — fails roughly 70% of the artists we quote. Variable income, multi-state networks, audiology, mental health, and the difference between a Silver and a Gold tier on tour: this is the full liner notes.

Why generic ACA advice doesn't fit musicians

Standard ACA guides assume a W-2 salary, one zip code, and one PCP. Musicians have none of those. Your annual income is a moving target, your office moves 200 miles a night during tour windows, and your specialists matter (audiologists, ENTs, laryngologists, therapists who get road life).

The four plan paths that actually work

  1. ACA marketplace with subsidies (the default for most 1099 players)
  2. PPO via marketplace or off-exchange (for touring acts)
  3. HDHP + HSA combo (healthy artists under 40)
  4. Small-group plan via band LLC (when there's W-2 payroll)

Estimating income honestly

Subsidies are based on annual estimated income — not last month's gig run. We help musicians estimate by stacking gig money, royalty estimates, teaching, sync income, and side W-2 work. Under-estimate and you'll owe at tax time. Over-estimate and you're paying more than you need to monthly.

When to pick a PPO over an HMO

If you cross more than two state lines per year for paid work, prioritize a PPO with a national footprint (BCBS, Aetna, United). HMOs work only in their service area, and an out-of-network ER bill on tour can be a tour-killer.

Can my band share a plan?

Not as individuals, but if your band runs W-2 payroll through an LLC or S-corp, you can set up a 2-life small-group plan with the band as the employer.

Are health premiums deductible on my Schedule C?

The self-employed health insurance deduction is taken above the line on Schedule 1, not on Schedule C — but only for the unsubsidized portion. Marketplace subsidies are not deductible.
